This should be typed on official letterhead and include the healthcare provider?s signature, credentials, and license number if applicable. ? Any copies of previous academic accommodations used either in high scool or college. ? Any other documentation related to the student?s disability such as psychoeducational evaluations or clinic notes. Handwritten notes on prescription pads are not considered reliable documentation.
