Documentation of a specific learning disability should consist of psychoeducational or neuropsychological evaluations performed by a professional qualified to diagnose and treat the disorder, including psychiatrists, clinical psychologists, neurologists, etc. Documentation should include the following: ? Diagnosis based on DSM-V or ICD standards ? Date of last evaluation or last contact ? A complete assessment of learning accounting for the following areas: Intellectual/Aptitude, Academic Achievement, Information Processing, including scores ? Summary of assessment procedures and evaluation instruments used to determine the diagnosis ? An interpretation of evaluations, as appropriate ? Information regarding functional limitations or barriers presented in major life activities, especially as they relate to a post-secondary environment (that may include a classroom, dining hall, residence hall, etc.). ? Recommended reasonable accommodations that will provide effective access to the student's university experience ? Any other relevant information, as appropriate. This could be information pertaining to history of other disability/impairment, relevant side effects of prescribed medication, etc.Documentation Required for ADHD
